Submit imaging for review for a remote consultation with Dr. Gilete

Upload Imaging – Evaluation and Consultation via Zoom

With this option you may submit scans completed at an upright imaging center close to you for Dr Gilete´s review and then connect via Zoom for consultation with Dr Gilete.

What imaging should be included?

Upright MRI of the occipital-cervical junction and cervical spine in:

    • Maximum flexion (sagittal cuts)
    • Maximum extension (sagittal cuts)
    • Maximum rotation, both sides (axial cuts)
    • Neutral (sagittal and axial cuts)

T2 imaging is preferred. Positioning should not provoke serious symptoms. Odontoid peg and surrounding region should be visible in all imaging. Neutral imaging should show hind brain.
All submitted imaging must be submitted in DICOM format.

Please be aware that the majority of the upright clinics in North America do not offer rotational imaging. In this case, CT scans in left, right, and neutral (C0-C3 axial cuts with 3D reconstruction) would be necessary to investigate for AAI. Please be aware that this represents a very high exposure to radiation. 

Additional imaging that may also be helpful

  • Cervical lateral cineradiology or DMX (flexion and extension and lateral bending) or
    • Cervical lateral x-ray in flexion, extension, neutral
    • Full spine x-ray in anteroposterior and lateral
    • Any additional spine or brain MRIs or CTs that are less than two years old  

Where can I have imaging done?

Imaging in the upright position (weight-bearing) is crucial to diagnosing instability. There are not many upright MRI clinics throughout the world with this technology. It is important to verify that that the clinic is a truly upright (not just “open” MRI) clinic using FONAR machines.

What are my options if I cannot obtain upright imaging?

Upright imaging is always preferable as a weight-bearing position is where instability is most apparent and thus measurable and diagnosable. However, some patients are not able to undergo upright imaging due to being completely bedbound. Please click HERE for more information regarding supine imaging.

Please be aware that supine imaging is inferior for diagnosis purposes and may not allow us to make a full evaluation.

What is the next step after I have obtained my imaging?

If you have all the requested imaging, please use the form here below and we will send you an email with a link for you to upload your imaging to us. Once all scans are correctly completed and uploaded, you will receive a proposed date for the Zoom consultation with Dr Gilete to review his findings, ask questions, and discuss further treatment options. You will also be sent the payment link. You have the option to request a report from the visit.
Before the teleconsultation takes place, you will be asked to complete
a set of comprehensive questionnaires from which we will gather your personal information, your clinical history, and symptomology.
