If you request an evaluation for CCI/AAI, investigation of Chiari/ectopic tonsil descent is included. If you suspect Chiari and have CCI symptoms, we recommend that you undergo the same imaging as for craniocervical instability.
Chiari is a condition where the hindbrain descends into the fossa space of the back of the skull and into the spinal canal. Chiari is diagnosed when tonsil decent prevents proper flow of CSF fluid. Patients with connective tissue disorders often have significant tonsil decent but this doesn’t always warrant a Chiari diagnosis.
An upright MRI of the brain is preferable but a supine brain MRI may also be used.

Surgery should always be the last option. However if symptoms significantly affect quality of life, a surgical option could be considered. Posterior Fossa decompression is a surgical procedure that removes a section of the bottom of the skull to allow more room for the tonsils.
